Black River Health and Jackson County Public Health are teaming up with Great Rivers United Way, health departments, healthcare systems, schools, and other community organizations for the Compass Now 2027 Community Health Assessment survey. This survey is done every three years to find out what the community needs and what health problems people are facing. Jackson County Public Health and Black River Health are asking the public, including you, to take the Compass Now survey to help plan community projects to create a healthier community.
Survey results are used to develop important programs, activities, and policies by local hospitals, health centers, and health departments. It is also used to apply for funding and grants, creating policies, and community development projects by community partners, and state and local leaders.
“The survey is very important in helping us understand what issues our community members are facing. This is why we want to hear from as many people as we can,” said Jordan Simonson from Black River Health.
The most recent Community Health Assessments helped shine a light on several challenges facing Jackson County residents, including housing and access to specialty healthcare.
In response to the findings, Jackson County brought a Community Health Worker into the community to help residents navigate housing challenges, connect with resources, and overcome barriers that can make it difficult to find and maintain stable housing.
Black River Health has used assessment findings to focus on improving access to specialty healthcare, working to bring more services closer to home so residents can receive the care they need without having to travel outside the community.
“Having current, local data helps us understand what our community is experiencing and, most importantly, where we can make a difference,” said Mollie Severson from Jackson County Public Health “The Community Health Assessment gives our partners a shared understanding of the challenges facing our residents and helps turn those findings into action.”
To take the survey, go to www.compassnow.org or visit Jackson County Public Health or Black River Health to get a paper copy.

The survey is available online in English and Spanish. The survey will take about 20 minutes to complete.
